Ojai Valley Community Hospital, an affiliate of Community Memorial Hospital is a 25-bed general acute care facility designated as a Critical Access Hospital with a distinct part 66 bed skilled nursing facility. OVCH provides inpatient, outpatient, and skilled nursing services for mostly primary and secondary care needs. In addition, we operate a 24-hour standby emergency department.

Work to maximize and integrate a Behavioral Health program for patients who are considered high-risk for medical and mental health issues including but not limited to depression, anxiety, substance abuse disorders, diabetes, and chronic illnesses. Ability to screen for mental health issues. Provide appropriate coordination of resources, referrals, and support in an effort to meet the total needs of the patient. Offer tools to assist patients with successful self-management interventions. Work in collaboration with medical team, community resources, and outside referrals.Qualifications
- Masters of Social Work (MSW)
- Current California Licensed Clinical Social Worker (LCSW) license
- Minimum 2 years of experience in a healthcare setting
- Bilingual in Spanish
- Experience in continuity of patient care planning
- Crisis intervention
- Experience working with high risk population
- Experience providing case management
