An essential member of the medical team, the Administrative Care Coordinator helps to facilitate and navigate patient care. The Administrative Care Coordinator provides a wide variety of administrative responsibilities in support of the Franciscan Children’s medical team and providers.
1. * Oversees the schedule for all onsite and off site appointments.
- Maintains a list of onsite and off site appointments
- Coordinates a with Providers, RT and RN to identify staffing to ensure feasibility of schedule
- Works with the outside clinics to schedule, reschedule or consolidate appointments.
- Coordinates COVID testing as needed
2. * Assists with onsite and offsite appointments.
- Supports parent participation in appointments as well as additional team members as needed
- Sets up technology and ensure patient and provider are present for the appointment
- Obtains results and documentations from visit and scan into EMR
- Maintains a list of providers involved in each patients care with contact information and need for further follow up
3. * Coordinates the care provided by consultants {GI, Renal, PACT, Pulmonary bronchoscopy, and ORL)
- Create schedule, obtains notes and get scanned into EMR
4. * Works with the medical team to identify ways to improve offsite, onsite and telehealth appointments for patients and families.
5. * Coordinates the discharge of patients.
- Maintains discharge checklist and send out reminders to keep process on schedule
- Works with providers and case managers to ensure that patients have appropriate follow up in place at discharge
- Supports the discharge medication process: Follow up with pharmacies, need for clarification, prior authorization, parents bringing in medications for review
6. * Coordinates the NP schedule to support adequate staffing for medical units
7. * Maintains records for NP orientation and ongoing competencies
EDUCATION AND EXPERIENCE
- High school diploma or GED is required.
- Bachelor’s Degree in business administration, health care administration, or related field preferred; or equivalent combination of education and experience.
- A minimum of 2 years of experience in a medical or healthcare setting.
KNOWLEDGE, SKILLS, AND ABILITIES
- Proficient with Microsoft excel, word, outlook, organizational and communication skills, comfortable with technology and online communication platforms
