Care Coordinator

Job Description

BHN is currently seeking Care Coordinators to work in our Care Management Programs. BHN provides team based, wrap around, whole health care management to adults and children in Integrated Care Management programs. Teams of Care Coordinators, RN Care Managers and Clinical Care Managers work closely with Primary Care Providers and specialty care providers to ensure that individuals receive the behavioral health and medical services and supports that they need.

The requirements for this position are as follows:

  • Provides high quality care coordination services, as well as family and collateral consultation on assigned cases.
  • Provides telephonic and in-person Care Coordination based on sound clinical theories and principles.
  • Includes all appropriate members in the individual's Integrated Care Team, including primary care providers, therapists, and others as indicated by the person served.
  • Maintains direct service productivity as agreed.
  • Monitors the effectiveness of Integrated Care Plans and coordinates with One Care team to make changes as needed to facilitate positive health outcomes and prudent use of resources.
  • Maintains flexibility in providing services in settings other than Clinics; e.g. client homes, health clinics, and other community settings.
  • Demonstrates cultural competence in dealing with clients with a variety of different cultural backgrounds.
  • Demonstrates working knowledge of community resources, and how to help clients access them.



Job Knowledge:

  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
  • Must have familiarity with internet, word processing and emails systems.
  • Professional demeanor and phone skills in communicating with members and care team members.

Experience and Training:

  • Minimum of three years experience working with the above populations.

Schooling or Equivalent:

  • Bachelor's degree OR equivalent work experience in related mental health field.
