We are looking for a DC Licensed Nurse or DC Licensed Clinical Social Worker to join the Case Management team within a top hospital in DC!!! This individual will help support the Case Management Team to prevent patients from re-entering the ER or the Hospital in general after they are discharged.
Once the patient is discharged, they will be engaging the patients on the phone to help prevent them from going back within 30 days. This will be calls to them, visits if need be, asking them if they have been doing their exercises or taking their medicine, or anything the patient may need. They want to make sure that they solve preventable re-admissions.
In order to be considered for this position you must wither be a DC Licensed Nurse or DC Licensed Case Worker with discharge planning or utilization management experience! This person must also have experience within a critical care environment.
