Job Description

PURPOSE
To support the company’s Mission, Vision and Core Values by providing direct care and support to individuals served promoting their health and safety and optimal well-being through the delivery of quality services.

ESSENTIAL DUTIES AND RESPONSIBILITIES
•Complete training on the needs of all persons supported that staff are assigned to work with.
•Ensure person supported specific goals and objectives are followed and addressed through implementation and documentation of the person supported’s programs and therapy plans.
•Be able to assist and support based on the specific needs of the persons supported.
•Accompany and supervise persons supported on outings.
•Support persons supported in their use of personal funds.
•Plan recreational activities.
•Follow all speech, dietary, therapy and behavior plans.
•Work direct care shifts, as scheduled and required. Shifts, hours and work locations are subject to change based on the needs of the program and as specified by your supervisor and/or management.
•Transport persons supported as required, based on the needs of the persons supported and the home/shift/work assignment.
•Ensure safe storage and preparation of food (well-balanced meals) in the homes.
•Ensure good health, hygiene, and grooming of persons supported.
•Complete and/or follow-up on the completion of documentation needed for person supported incidents, behavior tracking data sheets, accident reports, etc.
•Advocate on behalf of persons supported, where appropriate.
•Report medical and/or behavioral incidents or changes in health status and/or behaviors promptly.
•Ensure housekeeping is completed and cleaning checklists are filled out.
•Assist persons supported with housekeeping and other cleaning duties, i.e. cleaning their rooms, cleaning the bathroom after usage, cleaning up the kitchen and dining room after meals and snacks, laundry, etc.
•Complete Learning Logs and other service documentation completely and accurately, and follow-up on corrections needed.
•Complete required paperwork/forms concerning persons supported finances, sickness, daily activities, behavior, medication administration, etc.
•Attend persons supported ISP planning and COS meetings and provide input.
•Attend monthly residential staff meetings as directed by supervisor.
•Administer medications to persons supported as needed after successful completion of Medication Administration training.
•Provide transportation for persons supported as needed.

ADDITIONAL DUTIES AND RESPONSIBILITIES
•Represent the company in a professional manner in dealing with persons supported, family members, guardians, other agencies, and other outside professionals.
•Perform other duties as assigned.

PHYSICAL REQUIREMENTS
•Physically able to perform the essential functions of the job as described in the essential duties and responsibilities sections including; walking, bending, stooping, kneeling, squatting, sitting, reaching overhead and lifting up to fifty (50) pounds.
•Must be able to safely lift, transport, and transfer persons supported, which may include but not be limited to lifting and transferring in and out of a vehicle, bed or wheelchair, as appropriate and needed to assist with the person supported’s assisted daily living skills.

QUALIFICATIONS
•High school dipoma or GED desirable.
•Must have three personal references.
•Must have the ability to deal tactfully with others, to exercise good judgment, to understand and follow written and oral instructions, and to express ideas clearly, both verbally and in writing.
•Special consideration given to applicant with previous experience working with individuals with disabilities.
•Maintain valid driver’s license and meet D&S’ driver eligibility requirements if position/assignment requires driving.
•Must be able to successfully pass all required training.
