Director of Social Services - LBSW

Job Description

Carondelet Place (formerly Carondelet Manor), an Ascension Living community, has an excellent opportunity for an experienced Social Worker to join our team as the Director of Social Services.



Qualified candidates for this position will have, at a minimum, an LBSW, two years experience working in working in long term care environment preferably in a skilled nursing facility, and familiarity with discharge planning and documentation.




About Us:



Our skilled nursing community is conveniently located across the street from St. Joseph Medical Center in Kansas City, Missouri. When additional therapy is needed after surgery, illness or hospital stay, we provide rehabilitation and short-term care. We also offer long-term care for residents who need a higher level of assistance in dealing with an ongoing illness, chronic medical condition or disability.

POSITION SUMMARY


Manages, coordinates, directs, and maintains an efficient, productive, and organized social services department. Responsible for maintaining community compliance based upon regulations governing the community. Responsible for developing and adhering to departmental budgets within community guidelines. Functions as a member of the interdisciplinary team. 

Job Requirements

DUTIES AND RESPONSIBILITIES 


  • Works cooperatively and efficiently with Ascension Living, ED/Administrator, consultants, and community staff in planning, developing, implementing and evaluating of all social services. 

  • Assumes responsibility for identifying psycho-social situations, establishing goals for the highest practicable level of well-being and serving as a liaison between residents, families and community resources. 

  • Responsible for ensuring compliance with all policies and procedures and federal and state regulations pertaining to Social Services. 

  • Responsible for the social services component of the completion of MDS+, RAPs, Care Plan according to schedule and in participation with other disciplines.

  • Develops relationships with resident families, and encourage their participation in resident life.

  • Other duties as assigned.


EDUCATION AND/OR EXPERIENCE


  • Minimum Bachelor’s degree in Social Work or Gerontology required. 

  • Minimum two (2) years experience working in long term care; skilled rehab facility preferred  


CERTIFICATES, LICENSES, REGISTRATIONS 


  • LBSW or above by Behavioral Sciences Regulatory Board (BSRB) of state where employed preferred.
