Hospital Assessor Liaison

CenterLight HealthcareCHHA is an innovative managed care organization that excels in providinglong-term healthcare services is currently seeking experienced on-siteHospital Assessor for the Brooklyn region.


Position Summary: The primary purposeof your job position is to obtain required information and admit residents inan efficient manner, in accordance with established policies and procedures, asdirected by your supervisor. As the Hospital Assessor/Liaison, you aredelegated the administrative authority, responsibility, and accountabilitynecessary for carrying out your assigned duties.
 

Job responsibilities may include but are not limited to:  

  • Visit area hospitals on aconsistent basis while following the policies and procedures set by thehospital for encountering patients and accessing medical records
  • Identify the current needsof the facility to maintain a balanced acuity census
  • Establish and maintainrelationships with the Case Managers and Social Workers at the area hospitals
  • Submit pre-admissionassessment on each patient
  • Complete daily progressreport tracking referrals and admissions
  • Communicate withadmissions and nursing staff of facility prior to final approval of patientadmission
  • Report daily to assignedcoordinator total number of patient reviews and hospitals visited
  • Follow residentsdischarged to the hospital and communicate their final disposition
  • Identify any QA/QIconcerns that could affect admissions to facility
  • Maintain a log of dailyvisits with intent to report total visits to each hospital
  • Visit alternate level ofcare facilities at request to review prospective residents
  • Create and/or participatein marketing events
  • Assist in house admissionsstaff as necessary
