Job Description

Essential Job Functions: Major highlights of the position requirements are noted below.  Additional responsibilities may be assigned as required.
 
Referral Responsibilities:   • Handles all incoming referral calls and emails • Assess referral information, arrange for intake interview and make recommendation regarding referral eligibility • Request additional documents and intake material from the referral source to ensure the youth meets program criteria • Scheduled and conduct intake interviews (for all non-emergency referrals)  • Notify team members of the scheduled interview and share referral information • Make determination of program appropriateness • Complete Intake Packet at the time of admission.  • Provide administrative assistant Intake Information in order to create new case files.  • Transfer new resident successfully to the Parent Supervisor, SIL Program Supervisor and or his/her designee
 
Treatment Responsibilities: • Maintain frequent contact with Program Supervisors, Parent Supervisors, and Professional Parents and resource parents.  • Provide Intake Summary/Recommendations to treatment team. • Work closely with program staff to promote information sharing and/or challenges that arise during the intake process • Follow-up with youth within two (2) weeks of placement for any questions or concerns. • On call after-hour rotation for Foster Care Emergency Referrals 1 x monthly.   
 
 
Administrative Responsibilities:  • Maintain and keep daily record of referrals and other correspondence from referral source • Provide update to referring agency regarding the referral and the Intake status.  • Complete capacity reports for DHS and CUA.  
 
• Participate in 40 hours of In-Service training annually
 

Job Requirements

Education/Experience Requirements: This position requires an understanding of out-of-home care placement programs coupled with 2-3 years experience in recruiting, screening and assessing at-risk youth for program appropriateness. A  Master’s degree and valid driver’s license and insurance required.


Knowledge, Skills and Abilities:  • Proficiency in Microsoft Office, including Outlook and Excel  • Strong communication skills: ability to write well and communicate effectively, specifically over phone, through letters and email. • Personal characteristics: approachable, empathetic, flexible, discerning • Ability to work well as part of a team as well as independently. • Ability to effectively in a fast paced, team environment • Understanding of the Child Welfare System
