Position: LCSW Social Worker Manager, Case Management
Full Time - Monday-Friday 8:30a-5:00p

Job Summary
The Case Management Social Worker is responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situation, as well as disposition needs for each referred patient. Collaborates with patients, families, healthcare team members, and community agencies to develop and implement plans to address identified needs. Evaluates effectiveness of plans and initiates change as needed. Uses crisis intervention, problem solving model, community organization, and advocacy skills in identifying needs and resources in the hospital and community.
Minimum Job Qualifications
Licensed Clinical Social Worker required. CCM (Case Management Certification) or ACM (American Case Management Certification) required within 18 months of date of hire.
Eligibility requirements for certification: 12 months full-time case management experience supervised by a certified case manager, or 24 months full-time case management experience not supervised by a certified case manager, or 12 months full-time supervisory experience as a supervior of individuals who provide Case Management.
Demonstrated leadership in a formal management position. A minimum of 3 years of social work experience and 1 year experience in a hospital, agency or institution providing related health care services. Demonstrates the minimum knowledge, skills, and abilities to care for the individualized needs of the patient to include physical, psychological, sociocultural, spiritual and cognitive as well as functional abilities, including the need for diversified use of such practices.
