Summary:Provides patient care in the ambulatory care setting according to established standards of care, in collaboration with Care Managers and Medical Providers as members of the primary care team.

*Provides patient care in the ambulatory care setting according to established standards in collaboration with Medical Providers, Registered Nurses and Medical Assistants as members of the care team.*Provides primary support to the provider by proactively anticipating the providers needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures.*Ensures safe, timely, effective, efficient, equitable, and patient-centered care of patients.Qualifications

Other information:Minimum New graduate or experienced nurse from an accredited practical nursing program.  Education:Essential: High School Diploma or GED


Credentials:Essential:Licensed Practical Nurse-Other State


Competencies and skills:Essential:* BLSNonessential:* SKILL-Ability to effectively interact with customers to understand their needs and explain data* SKILL-Microsoft Office* Patient Safety Advocate: Works on behalf of patients, significant others, and family to represent concerns, and takes action to provide safe care. * Team Care: Participates on interdisciplinary team to meet patient care and clinic needs, working cooperatively with others to accomplish required work in a timely and efficient manner* Caring Practices: Provides safe, compassionate nursing care to promote comfort and healing and prevent unnecessary suffering or harm* Evidence-based Practices: Supports organizational and service line business objectives by following set standards of care and evidence-based practices that contribute to achieving optimal outcomes and patient experiences* Patient Education: Provides education for patients, significant others, and family using identified educational materials and resources under the direction of the medical provider* Response to Diversity: Recognizes and accommodates cultural, spiritual, gender, race, ethnicity, lifestyle, socioeconomic, age, and value differences and preferences in provision of care* Systems Resouces: Effectively uses environmental and system resources to provide safe, efficient and timely care to meet patient, significant others, and family needs* Clinical Reasoning and Actions: Uses clinical reasoning and established evidence-based guidelines for problem identification and resolution to provide patient care in a clinic setting. * SKILL-Demonstrated ability to communicate effectively in person and via telephone with members, employer groups, brokers, physicians, and physician office staff using strong dialogue and customer service competencies. * SKILL-Written communication
