Manage and coordinate patient care among high risk adults, veterans, or individuals with serious mental illness. Serve as primary link between the patient, provider, and Zero Suicide Program. Responsibilities are comprehensive in scope with emphasis on patient plan and ensuring necessary and appropriate least restrictive treatment and support is provided, including brief intervention, referral follow-up and transition of care. Facilitates the delivery of services and support is provided efficiently and in a timely manner. Maintains good working, patient/provider/staff relationships. Provides skilled intervention for the support and/or resolution of patient and family crisis, problem-solving and decision support. Complies with and enforces University Health System policies and procedures.
Master's degree in Social Work from a school accredited by the Council on Social Work Education is required.
Current state licensure from the Texas Department of Human Services is required.
