Job Description



Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities,behavioral health issues and substance abuse.With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware,Horizon House provides services to a diverse population focusing on individual strengths and choices.
We are seeking qualified candidates interested in joining our Assertive Community Treatment (ACT) team at our Wilmington,Delaware location.



This position is responsible to carry out the rehabilitation and support functions and to assist with treatment, provide substance abuse services, education,support and consultation to families; and to provide crisis intervention under clinical supervision.Some of the responsibilities include the following:



•    Provide service coordination {case management) for assigned case load,including monitoring the activities of the individual's treatment plan.


•    Evaluate and revise overall treatment goals and plans in collaboration with the
     participant and Team.


•    Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.


•    Provide symptom education to enable participants to identify their mental illness symptoms.


•    Provide direct clinical services including individual supportive therapy and psychotherapy to participants on individual,group and family basis.


•    Provide practical help,advocacy, coordination,side by side individualized support to help participants obtain the necessities of daily living including medical and dental care, legal and advocacy services and financial supports.

Job Requirements


      Masters degree in nursing,social work,psychology,rehabilitation, mental            health counseling or related clinical human service field,license preferred.



    Certified Alcohol and Drug Counselor {CADC) or Certified in the state of
       Delaware as a Certified Co­-Occurring Disorders Professional (CCDP)
       preferred.



                  Knowledge of mental health and substance abuse systems and supports.



   Ability to work independently.


                  Possess a valid driver's license with an acceptable driving record.
