Job description
Offers individualized therapeutic services and multi-level systems advocacy for patients who are high utilizers of hospital services to optimize timely access to quality medical, social and behavioral health services. Provides person-centered, holistic care that supports and empowers individuals, respects individual's choices and supports individuals on their road to healing. Understands and appropriately accesses community resources to address client's complex care issues.


EDUCATION: Master's degree in Social Work, Psychology, Counseling, or other related experience. Independent Licensure preferred including LISW and LPCC preferred.

CERTIFICATION/LICENSES: Must have a valid driver's license. BLS required.


  • Provide wrap-around care by partnering with clients to address physical, emotional, social and spiritual needs
  • Excellent communication skills with coworkers, providers and clients
  • Establish and maintain rapport with patients, families and other members of the health care team and community
  • Work effectively within an interdisciplinary team and promote a team approach to client care
  • Computer skills including Microsoft Office Suite and ability to enter data into spreadsheets
  • Work independently and to be self-motivated to meet with and assist clients in resolving complex care issues
  • Generate supportive group ideas and facilitate substance abuse recovery groups and individual clinical intervention in Sobering Center
  • Timely and thorough completion of client notes, documentation and files
  • Working with clients with complex care issues such as behavioral health, substance abuse, homelessness and domestic violence
  • Perform chemical dependency consults and domestic violence consults
  • Relationship building with community members and providers to create and strengthen continuum of care
  • Provide wrap-around care by partnering with clients to address physical, emotional, social and spiritual needs

EXPERIENCE: 2 years' experience working with marginalized populations


-Responsible to: Manager, Care Connection and Director, Community Health

ENVIRONMENT: General office environment, work in the community at various public locations, contact with patients in a hospital, long term care home and clinic settings. Potential for VDT exposure.

PHYSICAL REQUIREMENTS: Ability to move around the setting of care; requires close work, good vision, dexterity to write as well as to use computer keyboard; must be able to drive and travel within service population area.
