Job Description




  • Assists consumer(s) to implement Individual Service Plan goals
  • Provides companionship and individual supports to consumers participating in recreational, leisure, and social activities
  • Assists in adapting activities and materials according to individual needs
  • Provides one-on-one tutorial assistance as needed
  • Accompanies the consumer to community activities of his/her choice
  • Assists consumer(s) with daily needs which may include transportation, transferring, toileting, grooming and feedin
  • May implement behavior interventions with support from behavior specialist
  • Follows through on program details as required
  • May collect and record data
  • May produce progress notes, periodic reports or other forms of recordkeeping
  • Responds appropriately to input and direction from other team members
  • Demonstrates initiative.8. Attends and participates in staff meetings, individual supervision meetings and trainings as assigned; may be required to attend off-site trainings, staff meetings and conferences
  • Performs other functions as directed by supervisor, CPNJ management, or funding source representatives.




  • QUALIFICATIONS:

    High school graduate; at least one year’s experience in a non-profit service delivery organization or an equivalent combination of education and experience; demonstrated ability to engage in strenuous physical activity; fingerprint examination and approval; valid driver’s license; successful TB screening.


    Valid New Jersey Driver’s license (Standard) - Required upon hire


    Pass CPNJ Driver Training & Testing


    4 points or less against driver’s license

    Prior to working with individuals and/or Prior to implementation of a behavior plan


    Positive Behavior Supports & Functional assessment (Via the Boggs Center)

    Required Within the First 2 weeks of Employment:


    Training Today-Online

    o Preventing Slips, Trips, and Falls

    o Bloodborne Pathogens

    o Diversity for All Employees

    o Substance Abuse in the Workplace

    o Effective Communication for All Employees


    DCF Modules

    o Child and Adolescent Development

    o Conflict Resolution

    o Impulse Control and Anger Management

    o Limit Setting and Boundaries

    Required Within the First 30 Days of Employment:


    Life Threatening Emergencies (Danielle’s Law)

    Required Within the First 90 Days of Employment


    2-day New Employee Orientation Program


    Cardio Pulmonary Resuscitation (CPR)

    o Must be recertified every 2 years


    First Aid

    o Must be recertified every 2 years


    AED Training

    o Must be recertified every 2 years


    Elwyn Crisis Management Training

    o Must be refreshed annually


    Medication- College of Direct Support (CDS) Transcript

    o Medication Practicum-15 Days of completing the Medication Certification


    Shifting Expectation: Changes in Perception, Life Experience, & Services Certificate (College of Direct Support (CDS) Transcript


    Prevention of Abuse and Neglect College of Direct Support (CDS) Transcript

    o Prevention of Abuse and Neglect Competency Form

    ALL CERTIFICATIONS/TRAININGS MUST REMAIN CURRENT. FAILURE TO DO SO WILL BE CAUSE FOR TERMINATION.
