Job Description

Parkview Regional Hospital

Assists physicians and other medical personnel in achieving timely, accurate, and complete documentation practices. Handles correspondence relating to in-patient and out-patient medical records by:Handling requests for copies of patient records.Copying all requests for copies of medical records having reference to subpoenas (excluding malpractice).Maintaining a log of requests received and mailed out.Billing for all copy work performed that is billable, and billing for allowable charges for Medicare work.Logging-in and distribute to Physicians transcribed dictation referable to their patients.Photocopying required portions of visits to the hospital to be returned with client to other health facilities.

Assemble medical records of discharged patients by:Retrieving medical records of discharged patients on a daily basis from all nursing units.Checking loose report file for any dictation or reports that belong in current discharges.Checking each medical record for completeness, marking and attaching a deficiency slip.Preparing and numbering folders as needed.Updating locator file for record of each discharge patient.Transferring Master Patient Index (MPI) cards of discharged patients to permanent MPI.

Sorts, re-analyzes, reassigns medical records of discharged patients and performs deficiency actions by:Maintaining deficiency slips and deficiency logs.Sending deficiency letters regarding incomplete medical records of discharged patients to responsible physicians in accordance with the rules and regulations of the medical staff.Communicating with physicians' offices regarding medical record deficiencies and notifying them of up-coming suspension date. Complying with the hospital requirements, Joint Commission for Accreditation of Healthcare Organizations, State Department of Human Resources and Federal Regulations.

Handles and disperses all dictation by:Operating supervisor monitor and printer of transcriptions system on day or evening shift.Accounting for accuracy of monthly statements from the transcription bureau prior to presenting them for payment by accounting.Handling all dictation arriving by modem

Other duties as assigned by:Receiving and acting upon all special requests for studies, review, QA survey lists and other related requests.Ordering supplies and keeping supplies on hand at all times.Assisting Director and Assistant Director of Health Information Management in an active storage program.

Job Requirements

High school diploma or equivalent  X Required

Some medical terminology training  X Preferred

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

The Health Information Management Technician will be responsible for the Unit Record which contains both in-patient and outpatient encounters.
