Eckerd Connects Candor Short-Term Residential 

Eckerd Connects' Short-Term Juvenile Justice Residential program is a complete rehabilitative experience delivered in 4 to 6 months to 36 adjudicated male youth, ages 13 to 17, referred by the North Carolina Department of Public Safety Division of Juvenile Justice.  This residential treatment concept combines promising and evidence-based practices with a strong family transition component, and signifies a public-private partnership with the Division of Juvenile Justice which has produced an innovative shift in the way youth are served in North Carolina's juvenile justice system.  Intensive, short-term services include individualized treatment and academic plans that combine formal and experiential education, community service, behavioral health, and family counseling in a non-punitive environment designed to address the youth's behavioral challenges through a strength-based approach.  Youth also receive accredited education on-site and work together in small group settings with assigned counselors.

Position Concept
Provides clients with professional health services; to include: sick call, regular medical assessments, dental needs, daily triage of injuries and illnesses, record review, health screening and health education.

Essential Functions

  • Triage illness and injuries at each shift, determine medical priority, and treat within the scope of nursing practice; use physician standing orders, or referral when treatment is beyond then scope of nursing practice.
  • Monitor and/or administer medications, document adverse affects of the medication, and any test that may be required for each medication. As needed, obtain prescription re-fills in timely fashion to maintain client medication needs.
  • Conduct required health screenings including, but not limited to, height, weight, BMI, blood pressure, pulse, vision, hearing, acute and chronic illness evaluation, and substance abuse initial screenings. Screenings are to be conducted to meet or exceed state agency requirements.
  • Create and maintain Medication Administration Sheets for each client, monitoring for any missing signatures, medication errors, etc. Assure placement into correct MAR book, and file when completed.
  • During orientation and on an annual basis, staff members and youth will receive instruction on the exposure control plan, which includes standard precautions, bloodborne pathogens, and how communicable diseases are spread.
  • Coordinate with CQI and safety teams quarterly mock emergency drills on a number of emergency situations with detailed debriefing and critiques with corrective actions plans and follow up.
  • Coordinate with Clinical Director and psychiatrist, in accordance with state, agency, and federal guidelines the distribution, storage, and notification of psychotropic medication for youth.
  • Maintain “Controlled Drug Book”, assuring that agency required forms are being used and signed by staff when medication is administered and when nurse or cleared representatives are conducting shift-to-shift medication counts.
  • Conduct sick call at frequency to meet or exceed state agency requirements. Complete agency logs as needed (to include, but not limited to sick call log, episodic care log, sharps count log, over the counter medication inventory log, and TB testing log). As agency log requirements may vary from state to state, the RN will ensure that the logs required for a specific location are maintained.
  • Provides staff training as required by state agency. Examples of training may include, but is not limited to first aid, universal precautions, use of epi-pens, and medication review for those staff cleared to monitor self-administration of medications. CPR training may also be provided, if the nurse is a certified CPR trainer.
  • Systematically and periodically reviews client records and health care needs noting; but not limited to:
    • Immunization status, follow state guidelines
    • Past medical history/need for follow up
    • Laboratory studies/diagnostic tests, as ordered by physician
    • Current health care problems with special emphasis on chronic conditions
    • Medication and/or food allergies
    • Physical exam findings
    • Dental and visual Needs
    • Vital signs, BMI, as needed
  • Document health care progress, problems, needs, and recommendations according to established charting procedures.
  • Conduct check for injuries on youth after a restraint or other incident .
  • Prepare and submit health service reports as required by state or local agencies.
  • Monitor all off-site medical treatment forms; follow-up on doctor's orders.
  • Act as a liaison with the medical community.
  • Participate in treatment team staffing as needed.
  • Advise social services staff (e.g., Family Workers, Group Treatment Coordinators) of necessary health services.
  • Provide training and assistance in areas of infection control and OSHA compliance (i.e., blood borne pathogen standards, etc.) as needed.
  • Review all workers’ compensation First Report of Injury forms and provide first aid or physician referral as required.
  • Maintain health/first aid supplies, including, but not limited to the ordering epi-pens and bandaging units, ordering and stocking other supplies, monitoring expiration dates, and monitoring/maintaining of Automatic External Defibrillator (AED).
  • As required or assigned, transport clients to medical appointments.

Other Duties Include, But Not Limited To

  • Participate in training and development activities as required to maintain licensure (based on state board of nursing recommendations).
  • Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships.
  • Report any use of physical force and all unusual incidents per ECKERD policy and state guidelines.
  • Perform other duties as assigned.

Position Specifications
Education:  Requires Registered Nurse Diploma or degree. Must have and maintain current and appropriate RN licensure in the state where employed.
Experience:  Two years of school, correctional, doctor’s office, camp, industrial, or public health experience preferred. Experience working pediatric/adolescent patient care preferred.
Skills:  Requires discretion and judgment in making decisions. Requires ability to devise new methods or modifications, or adapting standard principles or practices to new or changed conditions. Must have working knowledge of child / adolescent related illness. Must be CPR certified. Specialized Knowledge - Familiar with the psychological / physical needs of youth in ECKERD related programs.
Other: Must maintain appropriate and valid state driver’s license. Must meet state criminal background check requirements.
Physical Demands:  Ability to lift/move up to 20 pounds and provide necessary physical services to clients Must be free of communicable diseases as defined by the state and to the extent that knowledge is attainable under the law. Must be able to transport clients.
