Social Worker/ RTF Case Manager

Job Description

The Residential Care SW has primary responsibility for developing, documenting, and implementing most aspects of a youth’s ISP during their participation in the CVCA RTF Treatment program; assures the safety, health, and welfare of the youth while using counseling, educational, and life skills to build character and encourage the youth’s successful achievement of their goals.
Responsibilities:


  • In coordination with the education and treatment teams, and in compliance with the Court Order and governing regulations, develops the youth’s individualized service plan for review and approval.

  • Maintains contact with and schedules sessions as indicated with individual youths, parents, DHS staff, and other authorized agencies and individuals

  • Conducts individual sessions with youth to discuss cause and effect of various behaviors; engages participants in an open dialogue concerning personal and family values, attitudes, and lifestyle; and counsels accordingly.

  • Establishes and maintains all legally required documentation pertaining to youth; creates a complete record of the activities and behavior of the youth and maintains the records in a confidential file.

  • Attends court hearings, DHS meetings and other required meetings; facilitates connections to other services that will support positive progress toward achieving goals.

  • Routinely reviews the records to determine progress and compliance with court ordered expectations, goals, and objectives of Life Plan; determines when modifications to the treatment plan are required, discusses changes with superiors and follows approved action.

  • Organizes and summarizes information provided at the monthly and quarterly reviews; attends and participates in meetings with family, treatment team members, DHS staff and other authorized agencies and individuals; follows up on recommended intervention; keeps parents apprised of progress, strategies for improvement, and parental participation in the program.

  • Functions as liaison between all parties associated with the youth's participation in the program; responds to questions and concern of youths; answers inquiries from parents, probation officers, and court management; attends team member meetings; briefs superiors on progress and concerns about individual cases.

  • Attends court hearings and reports on child’s progress

  • Observes and monitors the behavior of youths throughout their daily educational and recreational activities, determines compliance with the CVCA Behavior Management plan and takes appropriate action to recognize positive progress or sanction negative behaviors.

  • Prepares incident and progress reports; develops behavioral improvement plans and course of action to aid in the youth’s progress within the program.

  • Coordinates and supports discharge plans to promote a successful transition. keep all certifications 

Job Requirements


  • BS Degree in Human Services OR related field

  • Master’s Degree preferred




Knowledge, Skills and Abilities



  • Valid driver’s license from the state in which you reside in and proof of car insurance, daily access to vehicle.

  • Able to work on a team and independently

  • Knowledge of adolescent, mental and behavioral health

  • Good verbal and written command of the English language

  • Multi-tasking abilities  

  • Possess the ability to effectively communicate with children and adolescents

  • Contains significant frustration tolerance
