Manages patient's transitionthrough the healthcare system and transfers accountability to the appropriateperson or agency upon discharge.  Performs admission, concurrent andretrospective review for all patients using Interqual Criteria.  Providesnecessary information to 3rd party payers, physicians, staff, patients andfamilies.  Accountable for accurate resource utilization and facilitationof improvement in quality care for assigned caseload.  Provides servicesto patients throughout the lifespan including all age groups.

MinimumEducation:  Graduate of a Social Work BSW or MSW program.

MinimumExperience:  Must possess familiarity with case management issues,discharge planning and utilization management.

Licensure/Certification:  Mustpossess a current New Mexico license as a Social Worker.  InterqualCertification or Case Management Certification preferred.
