We proudly serve patients and families in private homes, assisted living facilities, skilled nursing facilities, hospitals, and our care centers located in Wilmington, Bolivia, and Whiteville, North Carolina. Our service area includes Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender Counties in North Carolina; and Horry, Georgetown and Marion counties in South Carolina. Our corporate office is in the coastal community of Wilmington, NC.

We are currently seeking a Social Worker to join our group of dedicated and experienced professionals.


This is an outstanding opportunity to join the largest and most experienced hospice in the area. Lower Cape Fear Hospice is certified by Medicare and Medicaid and approved by the Accreditation Commission for Health Care, Inc. Each day, more than 600 patients and their families receive care from Lower Cape Fear Hospice in our hospice and palliative programs.


Job Description


Knowledge, Skills and Abilities:


  • Assesses patient/family status to identify needs, provide supportive counseling, problem solving, service coordination, case management, discharge planning and placement within the scope of the social worker standard of practice.

  • Develops appropriate care plans in coordination with the patient/family, provides social work services as indicated, and assesses social work and team services in coordination with the IDT and in compliance with agency policies and procedures.

  • Educates the patient and family about how to access and utilize community services and benefit programs.

  • Acts as the liaison for the IDT with patient/family volunteers assigned to the SW’s patient/families.

  • Annually participates in continuing education to enhance professional practice.

  • Appropriately communicates information in accordance with LCFH policies and procedures.

  • Organizes and performs work effectively and efficiently.

  • Maintains and adjusts schedule to enhance team performance.

  • Demonstrates positive interpersonal relations in dealing with all members of the team.

  • Maintains and promotes customer satisfaction.

  • Effectively demonstrates the mission, vision, and values of Lower Cape Fear Hospice daily.

Job Requirements

Qualifications

Minimum Qualifications:

  • BSW from a CSWE-accredited School of Social Work required.

  • At least two years of professional social work experience required.

  • Preferred Qualifications:

  • MSW preferred.

  • North Carolina LCSW preferred.

  • Social Work experience in a health care setting preferred.

  • Location-Service Area: Bolivia, NC – Brunswick Care Center
