Social Worker

Job Description

  • JOB SUMMARY
    • Works under the direct supervision of the Administrative Director of Nursing. Provide medically related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident. Works with care management team to ensure provision of quality services along the continuum of care, decrease fragmentation, enhance the resident’s quality of life, efficiently use resident care resources, maximize cost containment opportunities, and improve successful transitions of care. Social Worker work is reflective of CCH mission, vision and values. The Social Worker will integrate departmental goals for improvement across the care continuum.



  • PRIMARY JOB DUTIES
    • Plans, directs and coordinates admissions by working closely with the potential resident, family, nursing staff and other facilities as indicated.

    • Plans directs and coordinates discharges to home or other facility as indicated by the residents plan of care, family, physician and/or facility staff.

    • Participates with the Interdisciplinary team in the resident assessment process, including but not limited to completion of MDS per facility and federal guidelines, coordinating overall plan of care meetings and writing and updating of the resident’s care plan.

    • Serves as a member of the facility leadership team.

    • Collaborates with the billing office in assuring compliance with regulatory guidelines related to utilization review, Notice of Non- Coverage, medical necessity, observation and admission status, etc

    • Collaborates and facilitates social service Quality Assurance program as directed by the Director of Nursing in conjunction with the facility QAPI program.

    • Coordinates the facility risk management team investigations for resident safety and continued monitoring that the facility adheres to Wyoming Department of Health regulations.

    • Coordinates communication with other departments, residents and families regarding continued social service issues including referrals to outside resources as needed to meet the residents’ needs.

    • Facilitates the facility complaint and grievance program in accordance with organization policy and procedures.

    • Maintains professional relationships with other departments, external organizations, service providers, physicians and families of patients.

    • Maintain skills through continuing education.

    • Must be free from governmental sanctions involving health care and/or financial practices.

    • Complies with the organization’s Corporate Compliance Program including but not limited to the Code of Conduct, laws and regulations and CCMH policies and procedures.

    • Perform other duties as assigned.

    • Coordinates all plan of care meetings (OPC and Baseline Care Plan Meetings).

JOB SPECIFICATIONS

  • Education: Bachelors degree in Social Work or human service field

  • Licensure: None required

  • Certifications required: See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.

  • Experience: Minimum 1 years experience in social service work

  • Essential Technical/Motor Skills: Ability to input information into the computer.

  • Interpersonal Skills: Excellent interpersonal skills in dealing with residents, family, co-workers and the public.

  • Essential Physical Requirements: Mobility sufficient to move throughout the facility.

  • Essential Mental Abilities: Ability to read and interpret data and to compile statistical reports, ability to assess and evaluate patients and potential problem situations.

  • Essential Sensory Requirements: Ability to visually assess and evaluate.

  • Exposure to Hazards: OSHA CATEGORY II. Tasks that involve a potential exposure to blood, body fluids or tissues. Identified job duties do not include a reasonable anticipation for the potential contact with blood or contaminated body fluid but circumstances may occur which may result in the potential for exposure. Use of appropriate protective measures is required for employee (s) engaged in potential exposure situations.

  • Hours of Work: Open 8-5 Monday-Friday, weekends or evenings may be required based on department functions.

  • Employees working in healthcare facilities have the potential to be exposed to hazardous materials including: Hazardous Chemicals/Drugs, Waste Anesthetic Gases, Radiation, Latex, Biological Hazards, Respiratory Hazards and Ergonomic Hazards. See Hazardous Materials in the Workplace Policy.
