Job Purpose: As a Senior Transition Counselor at Aspire Health Partners, your job purpose is to participate in the overall operation of the Intensive Psychosocial Rehabilitation program and in developing treatment that meet each client's needs. The job functions include - but not limited to - participates in the development of treatment plans, provide direct support individually and in groups to include activities of daily living skills acquisition, support groups and individual counseling when appropriate; maintain the integrity of the clinical record through timely and accurate completion of all required paperwork which includes treatment plans, assessments, progress notes, tracking forms and referrals; develop appropriate rehabilitation curricula for groups and activities scheduled within the program. In addition, this position will assist with Care Coordination for assigned clients, including SOAR applications and other individual client needs. As Transition Counselor, this position will participate in CSU Treatment Team and assist with identifying individuals who qualify for the Assisted Outpatient Treatment (AOT) order and in following up with AOT requirements.
Mostly importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to all company policies and procedures.
Position qualifications: Ability to receive and maintain a(n)
- Level II Background clearance
- Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire's insurance company as described in the AHP Corporate Transportation Policy 3.9.1
- Master's Degree in Social Services
- State of Florida License (LCSW, LMFT, LMHC)
Other Qualifications: Minimum of one year of experience; Must be able to work with a diverse populations of clients; Must be able to work independently; Good communication skills.
