KVC is seeking a Wrap Facilitator/Social Worker to facilitate the implementation of each family’s Service Plan by establishing contact with all providers identified on the plan, connecting providers with the family, maintaining ongoing communication with family and treatment providers, and coordinating care for the duration of services. The Wrap Facilitator/Social Worker will establish initial contact with families receiving FPRS services within 24 hours of referral to assess safety, initiate safety planning, and connect families with resources to any meet immediate needs. The Wrap Facilitator/Social Worker will conduct wraparound team meetings and maintain communication with the family and providers to monitor progress toward treatment goals and eliminate barriers to service provision and goal attainment. The Wrap Facilitator/Social Worker must maintain a flexible work schedule as necessary to meet families’ needs, be available to travel throughout the region, and participate in an on-call rotation.
The Wrap Facilitator/Social Worker must have a Bachelor’s degree in Social Work (or other related field) with ability to provide in-home services to children and families and experience working with complex and multi-need families.
Funding for this position provided through the Cabinet for Health and Family Services with Kentucky State Funds and/or Federal Funds.
Salary ranges are based on experience and have been increased to be more competitive.
