Eckerd Connects Wraparound Agencies
Eckerd Connects' Wraparound Agencies are part of the Coordinated System of Care (CSoC) under the state's behavioral health managed care program that serves children and adults in Louisiana with behavioral health, mental health and addictive disorders. Coordinated Systems of Care are national evidence-based models for coordinating services in a family-driven manner to ensure more positive outcomes for children, youth and families. The model incorporates a broad array of flexible effective services and supports that is organized into a coordinated network and that integrates care planning and management across multiple levels and systems. Through this coordinated network, meaningful partnerships are built with families and youth at service delivery, management, and policy levels. Family-driven and youth-guided services are provided in the home as well as in community settings that are data-driven and outcome oriented. Treatment plans are individualized and strength-based, culturally and linguistically competent, and integrated across all child-serving systems.

Facilitate a high fidelity wraparound process for youth and families, and work as a facilitator who provides individualized, coordinated, family-driven care to meet the complex needs of youth who are involved with several child and family serving systems, who are at risk of out of home placement and who experience behavioral and mental health difficulties. Facilitate and coordinate a process that requires the youth, family, providers and key member of the families social support network collaborate to build a plan of care that responds to the needs of the youth.
- Serves as the primary contact person for the family and collaboratively supports the development of the Individualized Plan of Care.
- Initiates contact with the family within (3) calendar days of receipt of referral.
- Conducts face-to-face with the family within (7) business days of assignment to discuss:
- Wraparound goals
- Wraparound process
- Role of the referring agency
- Expectations of Family/Caregiver
- Expectations of child
- Development of the Child/Youth and Family Team (CFT)
- Client/Family/Caregiver safety issues
- Creation of a crisis plan
- Stability of housing, childcare, and respite needs
- Works with Independent Evaluator (Licensed Mental Health Professional) to ensure that the comprehensive assessment is completed and the results are incorporated into the CFT process.
- Creates, convenes, coordinates and facilitates a Child and Family Team for each enrolled child/youth and family.
- Formalizes, monitors and modifies the Plan of Care developed by the wraparound team.
- Assures that families have a choice, sustained access to, and ownership of their Plan of Care and associated resources.
- In partnership with the family, develops an individualized Plan of Care that adheres to National Wraparound Initiative standards and treatment planning requirements consistent with 42 CFR 438.208(c)(3).
- Submits the Plan of Care to the Statewide Management Organization (SMO) for review prior to the end of the initial 30-day authorization period.
- Convenes, coordinates and communicates with the individual and identified collateral contacts to implement the Plan of Care.
- Works directly with the child/youth and family to implement elements of the Plan of Care.
- Coordinates the delivery of available services, including services reimbursable under 42 USC 1396d(a) educational, social or other services.
- Develops, in concert with the individual and collateral contacts, a transition plan when the child/youth has achieved the goals of the plan of care.
- Collaborates with other service providers on the child/youth and family’s behalf.
- Performs periodic reviews and updates of the Plan of Care to reflect the changing needs of the child/youth.
- Monitors the Plan of Care to ascertain whether services are being provided in accordance with the Plan of Care and are adequate.
- Demonstrates ongoing awareness of the community resources that could potentially be useful to the child/youth and family.
- Identifies other community and individual supports that can be used by the family to achieve their care plan; these can include friends, relatives and or religious/community affiliations.
- Develops positive relationships with community-based providers in order to ensure access to quality services for children youth and families.
- Ensures than an established community crisis support service is available to family during times of crisis, regardless of time of day or day of week.
- Assists the child/youth and family with accessing strength-based mental health, social services, educational services, and other supports and resources required to assist the family to attain its vision.
- Complies with all required documentation and data collection in support of the program’s implementation and evaluation strategy.
- Attends meetings and training's regarding Louisiana CSoC implementation.
- Maintain confidentiality in all aspects of client (i.e. HIPPA, FERPA, P.L. 46:56) staff and agency information.
- Assure availability to families 24 hours/day, 7 days/week including weekends and holidays.
Other Duties Include, But Not Limited To
- Maintain a high standard of role model behavior for youth and families.
- Participate in training and development activities as required.
- Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships.
- Report any use of physical force and all unusual incidents per Eckerd and CSoC and SMO guidelines.
- Perform other duties as assigned or needed.
Education: Bachelor’s degree from an accredited college or university in a social, human services, or behavioral sciences field required. Bachelor’s degree in other fields will be accepted with a minimum of two years of full-time experience working in relevant family, children/youth or community service capacity.
Experience: Previous experience working in a relevant family, children/youth, or community service capacity required if degree is in a field other than human services. Obtain wraparound practitioner certificate within 2 years of hire date. Must maintain certificate once obtained, including being re-credentialed annually.
Skills: Knowledge of family systems theory and application; behavioral therapies theory and application; cognitive-behavioral therapy theory and application; pragmatic family therapies theory and application; and/or social skills assessment and intervention. Basic working knowledge of word processing and electronic communications (WORD and Outlook preferred). Database management skills adequate to enter and retrieve data. Intermediate writing and oral skills. Ability to work independently, self-directed, good judgment. Ability to balance multiple, diverse and conflicting priorities. Ability to create effective relationships with individuals of different cultural beliefs and lifestyles. Possess an understanding of child and adolescent development. Must interface effectively at all organization levels, with commitment to professional standards and personal development.
Physical Demands: Must be capable of extensive travel and driving. Must be able to provide 24 hour, 7 day a week on-call crisis intervention to youth. Must be able to work a flexible schedule that includes some nights and week-ends. Ability to transport youth, volunteers and others. Must be free of communicable disease as defined by the state and to the extent that knowledge is attainable under the law.
Other: Must maintain a valid and appropriate driver’s license. Must maintain a safe driving record. Must maintain and provide annual proof of appropriate auto insurance. Must have use of a vehicle able to conduct home visits with therapists. Must meet state criminal and Eckerd background screening requirements.
